Claim Requirements
Please ensure you have the following documents ready before making a claims
- Certified copy of identity document of the deceased.
- Certified copy of identity document of the claimant.
- Certified copy of death certificate
- Certified copy of marriage certificate or affidavit in case of a customary marriage.
- Police affidavit by the claimant stating that they were appointed or given authority by the deceased’s family to collect funeral benefits from Thuso Burial Scheme.
- Notification of death form (DHA-1663 A/ BI-1663).
- Police Report in case of unnatural death.
